We are so pleased that you have come to us.

To help you on your first visit we have prepare some pages for

you to fill out ahead. These pages will be emailed to you once you have booked your appointment.

What to expect at your first visit

On your first visit we conduct a thorough history and examination. All of this takes time, so we ask that you completely fill out your paperwork prior to your first visit. If you cannot complete the paperwork online, please arrive 15 minutes early.

Your assessment will focus on your area of concern as well as your overall state of your health. After the doctor diagnoses your condition, a discussion of treatment options and approaches follows, culminating with your first treatment if appropriate.

Please plan to spend an hour for your first visit. Subsequent visits range from 15-30 minutes.
